Certified Dementia Practitioners

Thirty-two employees who work in Woodland Haven, the Facility's memory care neighborhood, recently completed training to become Certified Dementia Practitioners through the National Council of Certified Dementia Practitioners (NCCDP). NCCDP focus is on exceptional comprehensive dementia education for health care professionals and front line staff. Providing this training to the caregivers of Woodland Haven demonstrates the Facility's commitment to continual enhancement of the care and services provided to all of our residents!

2015 Red Nose Day

To celebrate Red Nose Day on May 21, 2015, the Houghton County Medical Care Facility sold red noses to staff as a fundraiser for the local charity organization, 31 Backpacks. 31 Backpacks distributes food to identified children from the Keweenaw Peninsula’s local school districts to take home with them at the end of the school week on Fridays so they don’t go hungry over the weekend. Since Red Nose Day was designated to help lift children out of poverty, it was determined that 31 Backpacks was deserving of the Facility's fundraiser. The Facility also has Casual for a Cause Fridays in which staff donate $1 to wear jeans to work on Fridays. Every nine weeks the money raised is donated to a local charity/organization. The most recent nine weeks was also designated for 31 Backpacks in conjunction with the proceeds raised from Red Nose Day!

HCMCF Received MPRO's 2014 Governor's Award of Excellence

MPRO's 2014 Governor's Award of Excellence
Houghton County Medical Care Facility is pleased to announce it has received MPRO's 2014 Governor's Award of Excellence for achievements in reducing health care acquired conditions in nursing homes. To receive this honor, Houghton County Medical Care Facility not only improved the quality of resident care in clinical quality areas such as pressure ulcers, physical restraints, antipsychotic use, falls and urinary tract infections, but provided organizational improvement, and actively participated in the statewide Michigan Learning and Action Network and the National Advancing Excellence Nursing Home campaign.

"This award demonstrates Michigan's deep commitment to providing high quality, safe and efficient health care," said Snyder. "By using proven, evidence-based practices, these organizations are making sure individuals get the right care at the right time."

This year, 188 Michigan hospitals, physician practices, nursing homes and community coalitions were honored with the award, which recognizes participants for their dedication to improve health care quality and patient safety in Michigan. To be eligible for the award, participating organizations must have achieved, maintained and continually improved in specific and rigorous milestones.

"We’re very fortunate to be able to partner with Houghton County Medical Care Facility and other dedicated organizations across the state of Michigan," said MPRO President and Chief Executive Officer Robert Yellan, MPH, J.D, FACHE. "To be able to recognize these accomplishments with the Governor's office underscores the importance of these efforts to improve health care quality in the state."

The Governor's Award of Excellence was developed in 2003. The current award is based upon work from 2012-2014 in the following areas:

--Reducing health care associated infections

--Reducing health care acquired conditions in nursing homes

--Reducing adverse drug events

--Integrating care for populations and communities through improving care transitions and coordination

--Improving preventive care and early diagnosis through immunizations and screening

--Improving clinical quality measures associated with
improving cardiac health

--Improving clinical quality measures among acute care and
critical access hospitals

--Reducing disparities of care in cardiac health

The award criteria are based on the National Quality Strategy, Partnership for Patients and Centers for Medicare & Medicaid Services (CMS) Quality Improvement Organizations' 10th Statement of Work. The preventive care metrics also align with the Governor's Healthy Michigan 4X4 plan.

Award recipients were honored May 9, 2014 at the Eagle Eye Golf Course in Bath Township, MI.
